Fusion Privacy Notice

Diameter Health is committed to protecting the privacy and security of information you provide us through the use of the Fusion Web User Interface (UI). This Privacy Notice describes the information we collect from users of the UI and explains how we use and protect that information.   

Personal Information we Collect

For the purpose of this Privacy Notice “personal information” means data that may identify you as an individual. 

We collect personal information directly from you when you or your organization set up your user account and when you log in and log out of the Fusion UI. This information includes your name, telephone number, email address/username, password, and IP address.  

We also record your activity, including the time and type of actions performed, session key, target records, file downloads, hostname, and certain metadata.

The UI is intended for users and activities located in the United States and this Privacy Notice describes how we use and disclose your personal information pursuant to United States laws. We make no representation that the application is appropriate or available for use in locations outside the United States. By using this UI, you understand and agree that your personal information will be processed in the United States or in the countries where our vendors are located. 

Changes to Privacy Notice

We reserve the right to change this Privacy Notice at any time, and such changes will become effective immediately upon posting. As business needs change, we will review our practices regarding the collection and use of personal information and amend this Privacy Notice as appropriate. .
